Jim Gaffigan has undergone quite the physical transformation since 2023.

The comedian and actor, who celebrated his milestone 60th birthday on July 7, has lost 50 pounds since going on the GLP-1 drug Mounjaro in 2023.

He first went public with his decision in 2024 with his comedy special The Skinny, and has since openly discussed the impact it has had on his health, both physical and mental.

Take a look at the actor’s transformation over the years in photos, plus what he’s said about his medical decision after experiencing weight fluctuations for much of his adult life…

Why Jim had “low expectations” of Mounjaro

Speaking with People, Jim explained his initial hesitance about going the route of a GLP-1. “I had very low expectations because I did know someone that had tried it and they were like, ‘Oh no, I was just nauseous for a week.’ And then I thought my true joy of eating would overpower anything.”

Why Jim struggled with his weight from an early age

He noted that he’d dealt with fluctuations in his weight ever since he was in his 20s, and growing older especially impacted his ability to work the pounds off. “[I] would work out twice a day just to be thin so I could eat,” he told People. “And then as my metabolism slowed down, it became harder and harder, and then my knees started giving out, and then fatigue set in.”

Why fans accused Jim of being on Ozempic

In his 2024 special, the Unfrosted star joked about being confronted by people for being on Ozempic, then the most well-known weight loss drug. “People will come up to me and accuse me of being on an appetite suppressant.”

“They’re like, ‘You’re on Ozempic. Admit it, you’re on Ozempic.’ And I’m not, alright? I’m on a different one,” he quipped. “I’m on Mounjaro, which is better, right? Because it sounds like an Italian restaurant.”

Why Jim definitely likes that he’ll “live longer”

During an appearance on TODAY’s Fourth Hour in 2025, Jim noted that he definitely did see the health benefits pretty quickly. “It’s always good, you know, like, ‘The health benefit, I’ll live longer,’ blah, blah, blah, but I love it just because I feel better,” he quipped. “It is great. It’s beneficial.”

Why Jim has continued using Mounjaro out of paranoia

Speaking with TODAY anchor Craig Melvin on his podcast Glass Half Full in March 2026, Jim confirmed that he’s still actively using Mounjaro. “It’s weird. It’s just something that’s in the background of my life at this point where I will take it definitely because I’m still paranoid that I’m going to feel nauseous.”

Why Jim doesn’t want to just be the “funny big guy”

In a conversation with Men’s Health, he shared: “People used to always say, ‘If you lost weight, you’d lose your whole act.’ It’s so strange, because in some ways, we’re not the funny big guy, are we? That’s the role that I’m supposed to play. But also in some ways, that may be the avenue of our warmth.”
